To register with
the practice please complete your
medical card and bring it into the
surgery together with two forms of
identification. One must be a photo
ID for instance passport or driving
licence and the second must be a
utility bill or solicitors letter in
your name at your current address.
All newly registered patients will
be asked to complete a
questionnaire.
Medical treatment is available from
the date of registration. PRESCRIPTIONS
If you are on repeat medication, please bring a printout
from your previous surgery when you
register. You can be issued with
one month of your medication but
will then need to see a doctor
before you are issued with a repeat
prescription. Please allow 48 hours
for your prescription to be
prepared (longer if you want it to
be sent to a pharmacy).
ACCESS FOR
THE DISABLED
We are fully equipped with
facilities for the disabled with
access ramps, wide doors and a
toilet.
