Who do I see?
Clinical Pharmacist/GP/Nurse
Advice:
The surgery does regular review on your medication and if required we will notify you if we required you do do anything.
If you have a long term condition and it is the month of your birth, please go to the long term condition section of this website to fill out the online form.
You can either book an *online appointment with the Pharmacist, GP or Nurse depending on what medications are being reviewed or alternatively please phone the reception desk on 01903 785152 to arrange your appointment.

Please ensure you have had your annual bloods in advance of the appointment and bring all your medications with you to the review.
